fix(setup): never prompt for a Vaultwarden URL outside a terminal or the legacy vault path ... 62fac4847a

staging-validate hung until the runner deadline at "Vaultwarden URL not configured":
setup called the Bitwarden config prompt unconditionally and `read` blocked on stdin.
